Please arrive 15 minutes early to complete any paperwork, and bring the following:
- Insurance Card(s)
- Valid Photo ID
- List of current medication, including dosages
- Copays are due at the time of your visit. We accept cash, check & all major credit & debit cards.
- If you are unsure of your copay amount, we recommend contacting your insurance provider prior to your appointment.
- You can pick up your orthotics at the clinic you were seen at.
- If you’re unable to pick them up, then please ask our office to mail them to you.
- An ABN form stands for Advance Beneficiary Notice (ABN)
- We ask Medicare & Medicaid patients sign this prior to receiving certain services that they may not cover.
- This does not mean a denial of care – it simply means we are required to inform you in advance that a specific service may be your financial responsibility.
- Our team will walk you through the form & answer any questions before you sign it.
- We only require this to be signed once each year.
- Prescriptions are sent electronically to the pharmacy of your choice.
- Please have your preferred pharmacy name and location ready at your appointment.
- Refill requests can be submitted through our patient portal, or by calling the office during business hours. We can write the prescription down for you on a pad if needed.
- Please allow 48–72 hours for refill processing.
- Many of our providers provide outreach at various facilities across Montana, & provide surgical services at different facilities.
- Please see each provider’s profile on the website for an active list of which facilities they provide services at.

- Each provider's schedule may vary based on surgeries, outreach clinics, medical conferences, & time off.
- For the most up-to-date availability or to schedule with a specific provider, please call your nearest office directly.
- We require at least 24 hours' notice if you need to cancel or reschedule.
- Appointments cancelled without sufficient notice are subject to a $50 no-show fee.
- We understand that life happens — if something comes up and you weren't able to give us notice, give us a call and we'll do our best to work with you.
