This notice is to make you aware of important changes at Bozeman Foot & Ankle Clinic, P.C. Effective September 1, 2025, Dr. Storm joined the provider team at Big Sky Foot & Ankle Institute. Rest assured, Dr. Storm will continue to provide the same excellent care that he has provided the Bozeman community for 30+ years. He is available for both new and established patient appointments at the 1050 E. Main Street location and you can secure your appointment time by calling (406) 587-8478. We look forward to caring for you!

Summary of What This Means for You

  • Same Provider, Same Care: Dr. Tod Storm will continue to provide care at the Bozeman office as a member of Big Sky Foot and Ankle Institute’s provider team. 
  • Additional Locations: In addition to our Bozeman location, Big Sky Foot & Ankle Institute has locations in Butte and Anaconda. You are more than welcome to make appointments with a provider at these other locations if they are more convenient for you.  
  • New Practice Name: Beginning September 1, 2025, all billing and insurance claims will be processed under Big Sky Foot & Ankle Institute. You may notice this on your Explanation of Benefits (EOBs) or financial statements. 
  • Updated Demographics & Insurance Information: At your next visit, our front desk staff may ask you to confirm or update your personal information and insurance details to ensure Big Sky Foot & Ankle Institute has accurate records. 
  • Medical Records: If you would like a copy of your records or need to direct a records request, please contact medicalrecords@sageandatlas.com
