The good news: most cases resolve with conservative care. The key is understanding what's driving the pain and addressing it before a minor overuse issue becomes something more serious.
What Are Shin Splints?
Medial tibial stress syndrome (MTSS) is an inflammation of the muscles, tendons, and bone tissue surrounding the tibia — the large bone in your lower leg. The condition develops when repetitive mechanical stress overwhelms the bone and surrounding soft tissue's ability to recover. The result is localized pain along the inner or front edge of the shin, usually covering a stretch of several inches rather than a single pinpoint spot.
It's worth distinguishing shin splints from a tibial stress fracture, which can present similarly but requires very different management. A stress fracture typically causes pain at a specific, focal point that intensifies sharply with impact. If your pain is narrowing rather than spreading, it's time to get imaging.
Common Causes
Shin splints rarely appear out of nowhere. They are almost always the result of a load the body wasn't ready for. The most frequent contributors include:
- Sudden increase in training volume or intensity. Jumping from three runs per week to six, or adding hill work without a gradual build, is the classic setup.
- Hard or uneven surfaces. Running on concrete or paved roads transfers far more impact force through the tibia than a dirt trail or track.
- Worn-out or inadequate footwear. Shoes that have lost their cushioning or don't match your gait pattern leave your lower leg absorbing shock it shouldn't.

- Tight or imbalanced calf muscles. Poor flexibility in the gastrocnemius and soleus increases tibial load during push-off.
Symptoms to Watch For
Shin splint pain typically presents as a dull, aching soreness along the inner border of the shinbone. Early on, pain is worst at the start of a run and eases as you warm up, then returns afterward. As the condition progresses, the pain may persist throughout activity and linger for hours after you stop. The area along the shin may feel tender to the touch, and mild swelling is possible in more advanced cases.
RICE: Your First Line of Defense
For most mild to moderate cases, initial management follows the RICE protocol:
- Rest. Reduce or eliminate the activity causing the pain. Low-impact cross-training such as swimming or cycling can maintain fitness without continuing the stress.
- Ice. Apply an ice pack to the affected area for 15–20 minutes several times per day, particularly after any activity.
- Compression. A compression sleeve or wrap can help manage swelling and provide mild support.
- Elevation. Elevating the leg when at rest helps reduce any inflammation-related swelling.
Over-the-counter anti-inflammatory medications (ibuprofen or naproxen) may provide additional relief. Always follow dosing instructions and check with your provider if you have contraindications.
Stretching and Strengthening for Prevention
Once acute pain subsides, rebuilding the lower leg's capacity to handle load is essential to prevent recurrence. Incorporate the following:
- Calf stretches (gastrocnemius and soleus). Hold each stretch for 30 seconds, two to three times per day. Both the straight-leg and bent-knee variations target different muscle heads.
- Toe raises and calf raises. Strengthening the muscles of the lower leg improves their ability to absorb tibial stress.
- Arch-strengthening exercises. Short-foot exercises and toe-scrunching drills help build the intrinsic foot muscles that support your arch.
- Hip and glute strengthening. Weakness higher up the chain often contributes to overpronation and excessive tibial rotation.
Returning to running should follow a gradual walk-to-run progression. The general guideline is to increase weekly mileage by no more than 10 percent at a time.
The Role of Orthotics
For runners with flat feet, high arches, or significant overpronation, custom orthotics can be a game-changer. By correcting the alignment of your foot and distributing pressure more evenly, orthotics reduce the rotational stress on the tibia that underlies many cases of medial tibial stress syndrome.
