For the more than 37 million Americans living with diabetes, foot health is not a secondary concern — it is a central one. Diabetes is the leading cause of non-traumatic lower-limb amputations in the United States, and the overwhelming majority of those amputations are preceded by a foot ulcer that could have been caught earlier. A routine podiatric foot exam — often taking less than 30 minutes — is one of the most powerful tools available for preventing that progression.

Why Diabetics Face Higher Risk

Diabetes affects the feet through two primary mechanisms that work independently but compound each other dangerously:

  • Peripheral neuropathy — elevated blood glucose damages the small nerve fibers that carry sensation. Over time, patients lose the ability to feel pain, temperature, and pressure in their feet. A blister, a small cut, or a pebble lodged in a shoe can go completely unnoticed.
  • Peripheral vascular disease — diabetes accelerates arterial narrowing, reducing blood flow to the lower extremities. Poor circulation means wounds heal slowly, immune response is impaired, and infections can spread rapidly.

The combination is particularly dangerous: neuropathy removes the warning signal (pain), and vascular disease removes the healing capacity. A tiny wound that a healthy person would resolve in a week can become a deep, infected ulcer in a diabetic patient in that same timeframe.

How a Small Wound Becomes a Major Complication

The pathway from minor injury to amputation is well-documented and largely preventable. It typically follows this progression:

  • An unnoticed pressure point, blister, or small cut develops — often from ill-fitting shoes
  • The wound is not felt, so the patient continues walking on it, deepening the wound
  • Reduced circulation delays the immune response; bacteria establish themselves in the wound
  • The infection spreads to deeper tissue, fascia, or bone (osteomyelitis)
  • Tissue necrosis may require surgical debridement, partial foot amputation, or in severe cases, limb amputation

At each point in this chain, earlier detection changes the outcome dramatically. That's why annual (and often more frequent) podiatric exams exist — not to treat existing problems, but to catch them before they become irreversible.

What a Comprehensive Diabetic Foot Exam Includes

A diabetic foot exam performed by a podiatrist is systematic and thorough. Each component serves a specific diagnostic purpose:

  • Visual inspection — the entire foot is examined for skin breakdown, calluses, ulcerations, blisters, fissures at the heel, nail changes, and signs of infection (redness, warmth, discharge). Nail thickness and color are noted. Spaces between toes — where moisture-related breakdown commonly begins — are specifically checked.
  • Sensory testing (monofilament test) — a thin nylon filament (Semmes-Weinstein 10-gram monofilament) is pressed against specific sites on the foot. Patients who cannot feel the filament have lost protective sensation and are classified as high-risk for ulceration.
  • Vascular assessment — the podiatrist palpates pedal pulses (dorsalis pedis and posterior tibial) and may assess capillary refill time. Diminished or absent pulses prompt further vascular workup.
  • Skin and nail care — thickened nails are safely trimmed (a task dangerous for patients to perform at home if sensation is reduced), calluses are debrided, and dry or cracked skin is assessed and managed.
  • Risk stratification — based on findings, patients are categorized by risk level, which determines how frequently follow-up exams should occur.

Risk Classification and Exam Frequency

Not all diabetic patients require the same follow-up schedule. Risk classification helps target resources appropriately:

  • Low risk — intact protective sensation, adequate circulation, no deformities, no prior ulcers. Annual exam is typically sufficient.
  • Moderate risk — loss of protective sensation OR vascular disease, but no active wounds. Exam every 3–6 months is recommended.
  • High risk — loss of protective sensation WITH vascular disease, significant foot deformity, or history of prior ulceration or amputation. Exams every 1–3 months are standard.
  • Active wound — weekly or biweekly visits until healed.

These frequencies are guidelines, not minimums. Patients should not wait for a scheduled visit if they notice any new skin changes, sores, swelling, or discoloration.

Fitting Into Your Overall Diabetes Management

A diabetic foot exam is one part of a broader care team approach. Your podiatrist works alongside your primary care physician and endocrinologist to close the loop on diabetes management. Foot exam findings — particularly vascular concerns — are communicated to your care team and may prompt referrals to vascular surgery or wound care specialists.
