When a patient comes in with ankle pain, the first instinct — for both patient and provider — is often to think sprain. And often that instinct is right. But a meaningful percentage of persistent or unexplained ankle pain has a systemic origin. The ankle is a joint like any other in the body, and any condition that inflames, degrades, or restricts circulation in joints can show up there first — or most prominently. Understanding these connections can lead to earlier diagnosis of serious conditions and better outcomes overall.

Gout: Uric Acid Crystals in the Joint

Gout is one of the most common systemic conditions that produces acute ankle or foot pain. It occurs when uric acid — a byproduct of purine metabolism — builds up in the bloodstream and crystallizes inside joints. The crystals provoke an intense inflammatory response, causing sudden, severe pain that can be debilitating within hours.

The big toe is the classic site (podagra), but the ankle is the second most frequently affected joint in gout. A gout flare typically presents as:

  • Sudden onset of severe pain, often overnight
  • Intense warmth and redness over the joint
  • Swelling that makes the ankle appear grossly inflamed
  • Pain so severe that even light contact with bedding is unbearable

Gout is confirmed with serum uric acid levels and, definitively, with joint fluid analysis. Treatment involves anti-inflammatory medications acutely and urate-lowering therapy (allopurinol, febuxostat) for long-term management. Dietary modifications — reducing red meat, shellfish, alcohol, and fructose — also play a role.

Rheumatoid Arthritis: Symmetrical Joint Attack

Rheumatoid arthritis (RA) is an autoimmune condition in which the immune system attacks the synovial lining of joints. The feet and ankles are among the most commonly affected areas — RA affects the foot or ankle in up to 90% of patients over the course of the disease.

Key distinguishing features of RA in the ankle:

  • Symmetrical involvement — both ankles are typically affected, not just one
  • Morning stiffness lasting more than an hour
  • Warmth and boggy swelling around the joint rather than bony enlargement
  • Associated findings — small joint involvement in the hands, fatigue, systemic inflammation markers (elevated CRP, ESR, positive RF or anti-CCP)

Podiatrists who recognize early RA presentations can facilitate prompt rheumatology referral, which is critical — early treatment with disease-modifying agents significantly reduces joint destruction.

Lyme Disease: Migratory Joint Pain

Lyme disease, caused by the bacterium Borrelia burgdorferi transmitted through tick bites, can produce a form of arthritis — Lyme arthritis — that typically affects large joints, with the knee being most common, but the ankle being frequently involved as well. In Montana, tick exposure is a real risk for outdoor enthusiasts, hunters, and hikers.

Lyme arthritis features intermittent, migratory joint pain and swelling that may shift between joints over days or weeks. Unlike gout or RA, there is typically a history of tick exposure and possibly an earlier bull's-eye rash (erythema migrans), though the rash is absent in up to 30% of cases. Serologic testing (ELISA confirmed by Western blot) is the diagnostic standard.

Peripheral Artery Disease: Circulation That Falls Short

Peripheral artery disease (PAD) occurs when atherosclerosis narrows the arteries supplying the legs and feet, reducing blood flow to the tissues. The ankle is a key diagnostic site — the ankle-brachial index (ABI), comparing blood pressure at the ankle to the arm, is the standard screening test for PAD.

PAD can produce pain in the lower leg and ankle in two ways:

  • Claudication — aching, cramping, or fatigue in the calf or ankle that comes on predictably with walking and resolves with rest
  • Rest pain — in more advanced disease, pain occurs even at rest, particularly at night, and is relieved by dangling the foot off the bed

PAD is associated with smoking, diabetes, hypertension, and hyperlipidemia. Identifying it through an ankle evaluation can lead to interventions — lifestyle change, antiplatelet therapy, or vascular procedures — that reduce cardiovascular risk significantly.

Diabetes: The Double Threat

Diabetes deserves separate mention from PAD, though the two frequently coexist in diabetic patients. Diabetic neuropathy causes loss of protective sensation in the ankle and foot, which can produce unusual pain patterns — burning, electric, or hypersensitive sensations — as well as areas of complete numbness. Simultaneously, vascular involvement reduces circulation and healing capacity. This combination makes even minor ankle injuries serious events in uncontrolled or long-standing diabetes. Lupus and Psoriatic Arthritis

Two additional autoimmune conditions frequently affect the ankle:

  • Systemic lupus erythematosus (SLE) — joint pain and swelling affecting the ankles and small foot joints is common, typically non-erosive. Associated with butterfly facial rash, photosensitivity, and multi-organ involvement. ANA testing is the initial screening step.
  • Psoriatic arthritis — in patients with psoriasis (or even a family history of it), inflammatory joint disease can affect the ankle alongside characteristic features like dactylitis (sausage toes), enthesitis (inflammation at the Achilles tendon insertion), and nail pitting.

When Ankle Pain Warrants Systemic Investigation

A podiatrist will consider systemic causes when ankle pain presents with features that don't fit a simple mechanical or traumatic explanation:

  • Bilateral involvement without a bilateral mechanism of injury
  • Pain that is worse at rest or overnight
  • Associated systemic symptoms — fatigue, rash, fever, or unexplained weight loss
  • No response to rest, ice, and anti-inflammatory measures over 2–4 weeks
  • Known systemic disease (diabetes, autoimmune conditions)

In these cases, a thorough history, targeted lab work, and appropriate imaging guide the investigation. Our goal is never to simply treat the ankle — it's to understand why the ankle hurts.
